Xylon Health was first formed in 2019 by four partners from the medical, nursing and accountancy professions when sharing in their first joint project of leading a Primary Care Network (PCN).
Each member of the team has plenty of experience in the healthcare sector in their respective domains, and share a common tenet in "Pursuing Good Through Healthcare", particularly in the area of helping the aging community.
Doing good begins by resonating with the average citizen. We believe that fragmented healthcare system in Singapore is a key pain point for many patients and caregivers alike.
Today, Xylon Health continues in its approach of "Innovation and Collaboration with others" by leveraging information technology and data to address problems faced by a greying generation: to address patient and elderly immobility, manage chronic illness, and enhance end-of-life quality of care.
